Eligibility To be eligible to receive services under Advantage Smiles for Kids, a child: Must reside in Oregon, Idaho or Washington • Must be considered “at-risk” - i.e. a child/teen whom society deems the potential to develop anti-social behaviors if not provided assistance. • Must be considered “low-income”: a child with no visible means of financial support to obtain orthodontic or supporting specialty dental care. • Must have been examined and all necessary dental treatment completed. • Must demonstrate a positive attitude toward keeping his/her dental disease under control. • Must sign appropriate release of information form allowing ASK to verify information. • Must have support from a caregiver willing to participate in the child’s treatment through transportation assistance; in oral health procedures where necessary; and in some kind of monetary assistance for the treatment,regardless of how small, with willingness to participate in the development of a financial treatment plan. • Must have been referred by a participating dentist, social security agency, CASA program, school counselor, teacher, administrator, mental health professional, etc. The referring person/agency must be willing to assist the child through the process, ensuring the requirements are understood, and that the child gets a primary care dentist with evaluation and follow-up care.
• Must be enrolled and in good standing in an accredited school and be working and on track to graduate from said school. Exceptions to the rule will be accepted with an established, documented reason. • Orthodontic care must be provided
by a participating orthodontist approved by ASK
